SAGE Can Help!

If you're an older adult who needs help in the home, or a caregiver who needs guidance finding and navigating the resources available to care for an aging relative, including seniors with Dementia in the home, SAGE's Licensed Social Worker is here to help. We offer free counsel, direction and advocacy to individuals, caregivers and loved ones.

Our Licensed Social Worker Will Help with:

  • Prioritizing current needs and challenges at home and facilitate enrollment in:
    • SAGE Home Health Care Services
    • SAGE Spend-A-Day Adult Medical Day Care Program
    • SAGE Meals Delivery Service
    • LifeStation Medical Alert (see below)
    • SHIP (State Health Insurance Assistance Program)
  • Transportation
  • Utility assistance
  • Medical supplies
  • Budgeting assistance
  • Navigating community and healthcare resources
  • Guidance and referrals for Assisted Senior Living
  • Relocation / downsizing assistance
  • Navigating public benefit programs
  • Guidance on resources to address emotional wellbeing, such as:
    • Prioritizing your needs and finding the balance and courage to practice self-care
    • Psychological changes and issues related to aging
    • The demands of being a caregiver and coping with "Caregiver Burnout"
    • The unique sociological challenges older adults face as they age

Our Social Worker is available for in-person appointments.

Engaging the Next Generation

Community Outreach

Some of the most meaningful programming and activities we develop at SAGE involve young people in our communities. We regularly partner with individual students and local schools to develop volunteer programs to support the Meals Delivery Service and Spend-A-Day. Intergenerational activities are vital to provide seniors with connection and purpose, while teaching young people empathy and respect for older generations.

Ways Students Can Get Involved include:

  • • Food Drives
  • • Delivery of holiday cards, crafts and placemats through Meals Delivery Service volunteers
  • • Visiting SPAD clients - playing bingo, doing crafts, sharing stories
  • • Performances from student musicians and choral groups
  • • Sensitivity Training - an interactive program helping students understand aging challenges through hands-on experiences and time with Spend-A-Day clients.
